Education and Understanding: Familiarise yourself with your health condition, its symptoms, and treatment options. Understanding your condition can empower you to make informed decisions.

Goal Setting:

Set realistic, achievable goals for your health and well-being. This could include physical activity / meaningful movement, healthy eating changes, or mental health strategies.

Regular Check-ins with your SSM Coach:

Keep track of your symptoms, medication, and any changes in your condition. This can help you identify triggers and track your progress.

Support Network:

Build a support system that includes healthcare professionals, self management coach, family, friends and work colleagues. They can provide encouragement and support when needed.

Self-Management Strategies: Incorporate self-management practices such as stress / relaxation management techniques, relaxation exercises, and hobbies that bring you joy.

Healthy Lifestyle Choices:

Prioritise healthy eating, regular exercise, and adequate sleep. These factors can positively impact your overall health.

Communication:

Keep an open line of communication with your healthcare providers. Discuss any concerns or changes in your condition, and don’t hesitate to ask questions.

Flexibility:

Understand that your plan may need adjustments over time. Be open to changing strategies as your condition or circumstances evolve.

Mental Health Support:

Consider seeking support from mental health professionals if you’re struggling with feelings of anxiety or depression related to your condition.

Celebrate Progress:

Acknowledge and celebrate small victories in your management journey, no matter how minor they may seem.
