Circle only one or two tools you think will make a positive change to self-managing your health. Ask for help from your support circle if needed.
Step 3
Once you feel confident with those tools, do the same steps and choose another two or three tools. Repeat again when needed.

What is Social Prescribing
Social prescribing is relatively new to healthcare, but is becoming an important part of supporting people with healthcare needs by guiding them to local services in their area.
Social prescribers are based in GP Surgeries, so ask at reception how to book an appointment.
