When you click on each of the tools from 1-12, the videos are from the Pain Toolkit, simply because many people with long term health conditions will also experience pain and other painful health conditions. The Self-Management Toolkit has been adapted from the Pain Toolkit which Pete Moore authored in 2001.

There is also a *Self-management Virtual Reality Hangout where Pete has posted more self-management reminders and  meet others living with long-term health conditions. An opportunity to swap ideas.

The Self-Management Toolkit is a simple guide that gives you some handy tips and skills to help you understand and manage a long term condition better and get you back in the driving seat and on track.

Going it alone

Living with a long-term health condition can be tricky and you may need some help at times. Where do you get help from? Well, help and support can come from your family, friends, healthcare teams, work colleagues and the Self-Management Toolkit. It’s important to get the right support in place. Make sure members of your healthcare professional team are experts or familiar with the supported self-management concept.
