Below is a sample "Nature Prescription" that can be incorporated into your Supported Self Management Plan to help manage pain and support long-term health conditions. 

You can adjust the specifics according to your preferences and local environment.

Nature Prescription for Pain Management and Long-Term Health

If accessible, spend time gardening or caring for indoor plants. This can be a peaceful and fulfilling activity that promotes physical movement and emotional well-being. Whether it’s planting, watering, or simply enjoying the greenery, take the time to connect with the earth.

Nature Engagement (1 time/week):

Participate in a nature-related activity at least once a week. This could include birdwatching, visiting a botanical garden, attending a nature walk with a group, or volunteering for local environmental projects.

Connecting with Others:

Join a local outdoor group or find a buddy to engage in these activities. Social connection is important for emotional health. Share your experiences and support each other in your nature activities.

Additional Considerations:

- Hydration and Nutrition: Make sure to stay hydrated during outdoor activities and consider bringing healthy snacks to enjoy in nature.
