Assess Your Current Situation:

Take stock of your current health conditions, treatments, and how they affect your daily life. This self-assessment will help you understand where you stand and what aspects you want to address.

Develop Strategies:

Based on your goals and current situation, outline strategies that can help you manage your health better. This might include:

   - Getting a Supported Self Management Coach

   - Medication management

   - Nutrition and exercise adjustments

   - Stress management techniques (like mindfulness or yoga)

   - Setting up a routine that supports your energy levels and capabilities

Seek Support:

Identify the sources of support available to you, such as Supported Self Management healthcare providers, therapists, support groups, or even friends and family. Connecting with others can provide encouragement and accountability.

Monitoring my Progress:

Keep track of how your strategies are working. This can include journaling your how your goals and goals, mood, and the impact of various activities on your well-being. Adjust your plan based on what you learn.

Celebrate Small Wins:

Acknowledge and celebrate even the small victories along your journey. Each step you take towards managing your health is significant.
