How do I include ‘intention’ within my Supported Self Management Plan

Set Goals and Action Plans:
- Make sure your goals are realistic achievable. For example, instead of saying "I want to eat better," say, "I want to eat two servings of vegetables each day by the end of the month."
Understand Your Reasons:
- Think about why each goal is important to you. Write down your reasons to help you stay motivated.
- Ask yourself: What good things will happen if I reach this goal?
Create Action Steps:
- Break your goals down into smaller steps you can easily do each day or week.
- For example, if your goal is to be more active, your steps could be: "Join a gym, plan workout days, and ask a friend to come with me."
Use Positive Words:
- Say positive things to yourself about your goals. This can help you believe in yourself more.
